Since early 2018 the Practice has been working with local services including The team at the Whittington Hospital and Patients with experience to provide group consultations to patients with raised HBA1C and who are at risk of diabetes as well as patients with chronic disease.
The Practice became certified early this year after successfully completing the consultations and undergoing training.
This project has been led by Mariam Suliman Senior HCA and Diabetes Associate and Jaya Narayandu, Lead Receptionist.
