Registering in Person

To register in person, you will need to come in to fill out our registration pack and hand it in to our Patient Services team.

Out of Area Registrations

The Village Practice also gives the option to register for people who are not living within our practice catchment area, but still within the Borough of Islington.

It is important to note that by registering with the The Village Practice as an out of area patient, requests for home visits would need to be provided by the NHS services near to where an individual lives and would be accessed through ringing NHS 111. The Village Practice does not provide home visits to out of area patients.

Out of area registration is voluntary for GP practices, meaning patients may be refused because they live out of area. Every out of area registration will be screened by the practice before acceptance onto the list.

Non-urgent advice: Safe Surgery

Please note – we are known as a ‘Safe Surgery’ and as such do not require Proof of ID or Address for registration.

However we many still ask you for documentation solely for spelling/accuracy purposes, so do request patients to have their ID or address at hand just in case.

Non-urgent advice: New Patient Check

When registering online, we do require you to come to the practice to complete your registration, and to complete a mini health check. This should take no longer than 15 minutes.

Adults – Age 16 and over

Anyone aged 16 and over is deemed an adult.

Children – Age 0-15

We can only register children if a parent or guardian is registered with the Practice or is registering at the same time. We will need to see the following:

  • A copy of the child’s Birth Certificate showing the parent’s names
  • The child’s Red Book or a record of any immunisations they’ve had

As part of our ongoing commitment to your health, we use mobile numbers and/or email addresses to contact you regarding your direct healthcare.

If you would prefer us not to do this, please inform a member of our Patient Services team.

Homeless?

Homeless patients are entitled to register with a GP using a temporary address which may be a friend’s address or a day centre. The practice may also use the practice address to register them. You do not need proof of address or ID.
