New Patient Registration

The area from which we accept new patients is bordered by part of Stroud Green Road, Blackstock Road, Highbury Park, Highbury Hill, Drayton Park,  Holloway Road, Tollington Way and Tollington Park.

Please check with reception if you fall within this catchment area by providing your street name and postcode.

Existing patients may live just outside this area but no new patients will be accepted outside this area. Please inform reception of any changes to your address or telephone number.

When registering please ensure you bring with you, proof of address (utility bill/rental agreement,within last 3 months), name/address of your previous Dr., passport if you do not have a previous Dr., refugee or asylum seekers must provide original Home Office documentation.

If registering a child under 5 years of age you must bring in the red book or proof of previous immunisations.

Your registration will not be processed unless relevant documentation is produced.
